A senior cat who’s suddenly eating like never before, yet steadily losing weight, is showing one of the most recognizable patterns in feline medicine: hyperthyroidism, the most common hormonal disorder in older cats.

What Hyperthyroidism Actually Is

Hyperthyroidism develops when the thyroid gland, located in the neck, produces excessive amounts of thyroid hormone, which speeds up the body’s metabolism well beyond normal. It typically occurs in middle-aged to older cats, and it’s common enough that roughly one in ten cats age ten and older develops it. In the large majority of cases, the cause is a benign, non-cancerous growth on the thyroid gland rather than a malignant tumor.

The Telltale Signs

The hallmark pattern is weight loss despite a normal or increased appetite, which is a genuinely useful clue since weight loss on its own is often mistaken for a normal part of aging in cats when it isn’t. Other common signs include increased thirst and urination, patchy hair loss or a generally unkempt coat from reduced grooming, restlessness or increased activity, and sometimes vomiting or diarrhea. A noticeably faster heart rate is also common, since thyroid hormone directly affects heart function, and left untreated, hyperthyroidism can eventually strain the heart and other organs.

Diagnosis

Diagnosis is confirmed through bloodwork measuring thyroid hormone levels, most commonly total T4, which is elevated in the large majority of hyperthyroid cats. Because hyperthyroidism can also mask or complicate underlying kidney disease, a common concern in the same age group, vets typically run kidney function tests alongside the thyroid panel to get a complete picture before deciding on treatment.

Treatment Options

Several effective treatment paths exist, and the right one depends on a cat’s overall health, other conditions, and an owner’s practical situation. Radioactive iodine therapy is considered highly effective, given as a single treatment that destroys the overactive thyroid tissue without harming surrounding organs, though it requires a period of hospitalization at a specialized facility for radiation safety. Daily oral medication, methimazole, is a common alternative that controls hormone levels without addressing the underlying thyroid growth, meaning it requires lifelong use and regular monitoring. A specially formulated iodine-restricted diet can also control hyperthyroidism in some cats, provided it’s the only food they eat, and surgical removal of the affected thyroid tissue is another option in select cases.

Why Annual Testing Matters

Because hyperthyroidism develops gradually and its early signs overlap so heavily with normal aging, annual blood and urine testing is generally recommended for cats seven and older specifically to catch it, and related senior conditions, before they progress. Catching hyperthyroidism early generally means starting treatment before it has had time to strain the heart or complicate kidney function, which meaningfully affects the range of treatment options still available.

Hyperthyroidism and Kidney Disease: A Tricky Balance

One of the more counterintuitive complications of treating hyperthyroidism is that an overactive thyroid actually increases blood flow to the kidneys, which can mask underlying chronic kidney disease that’s been quietly present the whole time. Once thyroid hormone levels come down with treatment, blood flow to the kidneys normalizes too, and in some cats this unmasks kidney function problems that weren’t obvious before treatment started.

This is exactly why vets typically don’t rush straight to a permanent treatment like radioactive iodine without first checking kidney function, and why some cats start with a reversible option, like methimazole, specifically to see how their kidneys respond to lower thyroid hormone levels before committing to a permanent fix. It isn’t a reason to avoid treating hyperthyroidism, since leaving it untreated carries its own serious risks to the heart and other organs, but it’s part of why the treatment plan is built around your cat’s individual bloodwork rather than a one-size-fits-all protocol.

Cost and Practical Considerations

The various treatment paths differ meaningfully in upfront cost, ongoing effort, and long-term value, which is worth discussing openly with your vet rather than assuming one option is automatically right. Daily medication has a lower upfront cost but requires lifelong pilling and repeat bloodwork to keep dosing accurate. Radioactive iodine therapy costs more upfront and requires a temporary hospital stay, but it’s often described as effectively curative for the thyroid issue itself, meaning no more daily medication afterward. Dietary management avoids medication and hospitalization entirely, but only works if a cat eats nothing else, which isn’t realistic in every household. There’s no universally right answer here, only the option that fits a specific cat’s health picture and a family’s practical situation.
